General registration opens on June 6.


The Street Medicine Institute is pleased to announce that registration is open for the 21st annual International Street Medicine Symposium, to be held September 9 – 12, 2025 in Hilo, Hawai‘i. We are grateful to our local co-host HOPE Services Hawai‘i for helping to organize this symposium. 

The International Street Medicine Symposium (ISMS) is the world’s premier educational event dedicated to the health care of rough-sleeper and unsheltered homeless populations. Each year, ISMS offers an impressive slate of homeless health care experts from around the globe presenting clinical topics, innovations, research outcomes, and best practices relevant to street medicine through the popular format mix of large-group lectures, interactive small-group presentations and workshops, and poster presentations. All people working in the area of health and homelessness are invited, and we encourage and celebrate the diversity of lived expertise and professional roles represented among participants.

The symposium will be held at the Grand Naniloa Hotel Hilo on September 10 – 12, 2025, with an optional pre-symposium Street Medicine 101 workshop Tuesday, September 9.

On behalf of our Hilo ‘ohana and the HOPE Services Hawai‘i team, it’s our great honor to welcome you to the 21st International Street Medicine Symposium—hosted for the first time in Hawai‘i.

Hawai‘i Island is a place of healing, resilience, and deep connection. This is where community, culture, and care come together —and that’s what street medicine is all about.

At HOPE Services, we meet people where they are, bringing healthcare and support to those living unsheltered across our island. From beach parks to backroads, we show up with urgency, compassion, and aloha.

We don’t do this work alone. Our strength comes from our partnerships—with clinics, hospitals, faith communities, public health teams, and dedicated volunteers.

As we gather in Hilo, we invite you to share your mana‘o (wisdom), connect with the people and spirit of this place, and carry renewed energy into the work ahead.

SMI's Inclusive Registration Rates

The street medicine community includes individuals and organizations of all types, sizes, and budgets and we want all of you to be able to participate in the 21st annual International Street Medicine Symposium. The Street Medicine Institute (SMI) is offers three paid registration tiers to help enable our colleagues with recent lived experience of homelessness and our colleagues from countries that are offered free SMI membership to register for free.

As a non-profit organization dependent upon this income for our sustainability, SMI asks that you register at the price tier best reflecting your resources and the value you place on participation in the symposium. Paying at the highest tier possible for you allows SMI to include colleagues who have fewer financial resources. As you weigh the most appropriate tier for you and your team, please factor in that ISMS 2025 will include approximately 20 hours of educational content. Also, please remember:

Members of the Street Medicine Institute receive a 50% discount on ISMS registration and the Street Medicine 101 pre-symposium workshop.

Tier 1- Full-price Tier

This registration tier is suggested for physicians, administrators, advanced practice providers, and others from organizations that provide professional-development budgets for employees. This tier includes continuing education credit.

Tier 2

This discounted registration tier is suggested for other professionals who need continuing education credit and are paying their own way, or who work for an organization that cannot support a full-price registration. This tier includes continuing education credit.

Tier 3

This registration tier is suggested for students (including residents and fellows), outreach workers, and others who are paying for their own registration and do not need continuing education credit.

We strongly encourage you to register for the symposium before booking your symposium travel and accommodations. If ISMS sells out, we will be unable to add a registration space for you, even if you have already booked flights and a hotel.

Group Memberships

If you are registering a group of people from your program, please ensure they have first all been included in your Group Membership account. You may continue to add individuals to your Group Membership and register them for the symposium at any time until the symposium. Please contact [email protected] if you require assistance.

Accessing Free Registration

If you live in one of the countries in our International C group or have recently experienced homelessness, please become a member of the Street Medicine Institute for free at the beginning of the ISMS registration process. Once you sign up as a member, you will be able to register for the symposium for free.

The “People with Recent Lived Experience” category is right for you if you have experienced homelessness in the past five years and you are not currently in a professional role. If you are working in a professional role, we kindly ask that you register at the highest registration tier that you or your organization can support.


Street Medicine 101 Workshop

Symposium Add-on- Tuesday, September 9, 2025

The Street Medicine 101 pre-symposium workshop addresses common challenges among new and developing Street Medicine programs around the world. This high-yield introductory session is targeted to those individuals and organizations new to Street Medicine or in need of expert guidance to launch or enhance their Street Medicine activities. This workshop will provide an instructive overview of the history, principles, and practice of Street Medicine, as well as interaction with experts in the field for guidance and problem-solving.

Transportation

Air Travel

Hawai‘i Island has two main airports:
    • Location: Hilo is on the east side of the island
    • Distance to Symposium Venue: Approximately 3 miles (10 minutes by car)
    • Location: Kailua-Kona is on the west side of the island
    • Distance to Venue: Approximately 77 miles (2 hour drive)

Transportation from Airports

  • From Hilo Airport to Grand Naniloa Hotel:
    • Taxis, ride-shares (Uber/Lyft), and shuttle services are available.
    • Driving Directions: Exit the airport via Airport Road, turn right onto Kanoelehua Avenue, then follow signs to Banyan Drive.
  • From Kailua-Kona Airport to Grand Naniloa Hotel:
    • Rent a car for the drive across Saddle Road (Daniel K. Inouye Highway). This scenic route takes approximately 2 hours.
    • Shuttle services between Kona and Hilo are limited and may require advance booking.

Car Rental Information

Renting a car is highly recommended to make the most of your visit to Hawai‘i Island, as public transportation is limited, and the island has many attractions.
  • Hilo Airport Car Rentals:
    Rental car services are readily available at Hilo International Airport. Reservations are encouraged due to high demand.
  • Kailua-Kona Airport Car Rentals:
    Kona International Airport has several car rental agencies. However, a rental from Kona will require a 2-hour drive to Hilo, so plan accordingly.
  • Important Tips:
    • Reserve your car in advance to secure availability and the best rates.
    • If you plan to explore areas like Mauna Kea or Hawai‘i Volcanoes National Park, consider renting a four-wheel drive (4WD) or sport utility vehicle (SUV).
    • Paid parking is available at the Grand Naniloa Hotel for guests and attendees.
